目的

探讨超声引导双极射频感应温热疗法(RFITT)治疗下肢静脉曲张的有效性和安全性。

方法

选取2016年8月至2018年2月在北京友谊医院接受超声引导RFITT治疗下肢静脉曲张患者38例(共42条)治疗及随访资料,观察术后并发症情况,记录并比较治疗前及术后1个月、3个月、6个月的治疗效果。治疗前与治疗后静脉临床危重程度评分的比较采用配对t检验。

结果

对38例患者共42条肢体进行治疗,术后随访32例患者达到治愈效果,5例有效,1例无效,总有效率97.37%。术后静脉临床危重程度评分1、3、6个月呈进行性下降,术前评分为14.27±9.36,术后1、3、6个月评分分别为:9.20±6.87、7.86±4.02、4.53±4.22,与术前比较,差异均有统计学意义(t=1.834、2.267、2.682,P均<0.05)。随访过程中发现2例患者出现局部皮肤烫伤,3例患者术后局部皮肤感觉障碍,均在1~3个月内恢复正常,2例患者出现色素沉着,未能完全恢复。所有患者均未出现深静脉血栓及肺栓塞等严重并发症。

结论

超声引导RFITT治疗下肢静脉曲张疗效可靠,且安全、微创。

Objective

To evaluate the efficacy and safety of ultrasound-guided radiofrequency-induced thermotherapy (RFITT) in the treatment of lower extremity varicosity.

Methods

Forty-two varicose veins in 38 patients who underwent ultrasound-guided RFITT were analyzed. The paired t-test was used to evaluate the efficacy of the procedure at 1, 3, and 6 months after RFITT, and the side effects during and after the procedure were documented.

Results

Thirty-two of thirty-eight patients achieved clinical cure, three achieved partial cured, and one patient had no improvement. The total effective rate was 97.37%. The scores of varicosity before RFITT and at 1, 3, and 6 months after the procedure were 14.27±9.36, 9.20±6.87, 7.86±4.02, and 4.53±4.22, respectively, and a significant difference was observed at each time point after RFITT compared with the value before the procedure (P=0.032, 0.015, 0.007). During the follow-up, two patients had localized skin burn and three patients had sensory disorder, but all these patients fully recovered within 3 months. Two patients had pigmentation and did not obtain fully recovery at the end of the study. No major complications occurred.

Conclusion

Ultrasound-guided RFITT is an effective, safe, and minimally invasive therapy for lower extremity varicosity.
