近年来,我国孕产妇高龄化趋势日渐明显,不孕率也逐渐增高,辅助生殖技术以及促排卵药物在临床工作中的应用日趋广泛,我国双胎妊娠的比例逐年上升。相较于单胎妊娠,双胎妊娠发生围产期合并症及并发症的风险更高,双胎妊娠中双胎之一畸形或两胎畸形是双胎合并胎儿畸形常见的并发症之一,据国外文献报道,双胎合并畸形的发生率为4%~6%,是单胎合并畸形的1.7倍,各种主要的先天结构畸形在双胎中的发生率都高于单胎。据文献报道,在单胎足月活产新生儿中心脏畸形的发病率为6.5‰~10.0‰,而在双胎妊娠中则更为常见,双胎或双胎之一合并心脏畸形的发病率约为20‰。由于合并心脏畸形的双胎面临比单胎更高的妊娠风险,且临床处理更为复杂,因此,加强对双胎或双胎之一合并心脏畸形的产前筛查及监测,尽早发现心脏畸形的存在并进行必要的侵入性产前诊断或宫内干预治疗,对降低双胎妊娠围产儿死亡率及出生缺陷率至关重要。
