冠状动脉粥样硬化性心脏病(以下简称:冠心病)是临床上最常见的心血管疾病之一,严重威胁人们的生命和健康状况。准确评估心脏功能对于冠心病患者的早期诊断及预后评价具有重要意义。与传统超声心动图的左心室射血分数(left ventricular ejection fraction,LVEF)及室壁运动评分(wall motion score,WMS)相比,基于应变的组织多普勒或斑点追踪成像(speckle tracking imaging,STI)技术可以定量分析心肌形变,提供客观评估心功能的多参数信息。近年来,关于收缩早期延长(early systolic lengthening,ESL)和收缩后收缩(post-systolic shorting,PSS)这两种心肌矛盾形变在冠心病中的应用研究逐渐兴起并备受临床关注。ESL和PSS都是心室壁节段的细微异常运动,常发生于收缩功能障碍的心肌中,由于受损心肌与周围正常心肌间张力不平衡所致。心肌矛盾形变能够早期识别心肌缺血、准确评估心肌梗死,并为冠心病患者的疗效评估及预后判断提供重要临床信息。本文就ESL和PSS在冠心病中的应用进展进行综述。
