分析不同类型左头臂静脉异常胎儿的超声心动图特征,提高对该类疾病的诊断能力。
回顾性分析2019年7月1日至2022年3月31日于南京医科大学附属常州市第二人民医院胎儿超声心动图检查诊断且经产后超声心动图证实为左头臂静脉异常的55例胎儿的超声心动图表现,观察异常左头臂静脉的超声表现,将其分为左头臂静脉缺如、左头臂静脉走行异常及左头臂静脉管径异常,并总结其合并心脏异常情况。
55例胎儿中,左头臂静脉缺如46例,其中永存左上腔静脉44例,右侧上腔静脉缺如2例,46例中32例合并心脏异常为左心偏小;左头臂静脉走行异常7例,其中胸腺内走行2例,均合并法洛四联症,主动脉弓下走行5例,4例合并心脏大血管异常,1例无合并异常;左头臂静脉管径异常2例,2例均为左头臂静脉增宽合并肺静脉异位引流心上型。
超声心动图可有效识别多种类型的左头臂静脉异常胎儿及合并异常,了解各类型左头臂静脉异常的超声表现有助于提高其诊断率。

To analyze the echocardioimagedata characteristics of different types of left brachiocephalic vein abnormalities in fetuses and to improve the diagnostic ability for these diseases.
A retrospective analysis was performed on 55 fetuses with abnormal left brachiocephalic vein abnormalities confirmed by echocardiography at the Second People's Hospital of Changzhou from July 1, 2019 to March 31, 2022. According to echocardioimagedata findings, the abnormal left brachiocephalic vein was divided into absence of the left brachiocephalic vein, abnormal course of the left brachiocephalic vein, and abnormal diameter of the left brachiocephalic vein.
Among 55 fetuses included, the left brachiocephalic vein was absent in 46 cases; the left superior vena cava was persistent in 44 cases, and the right superior vena cava was absent in 2 cases. There were 7 cases of left brachial vein abnormality, including 2 cases of intrathymic vein abnormality, 5 cases of subaortic vein abnormality, 4 cases of left brachial vein abnormality, 1 case of left Tetralogy of Fallot vein abnormality. The diameter of the left brachiocephalic vein was abnormal in 2 cases, both of them were supracardiac type of anomalous pulmonary venous connection with widening of the left brachiocephalic vein.
Echocardiography can effectively identify various types of left brachiocephalic vein abnormalities in fetuses and associated abnormalities, and understanding the ultrasonoimagedata features of various types of left brachiocephalic vein abnormalities is helpful to improve their diagnostic rate.
