Diagnostic value of emergency bedside echocardiography in cardiovascular diseases

Abstract:

Objective

To objectively analyze the diagnostic value of emergency echocardiography for cardiovascular diseases (CVD) by receiver operating characteristic (ROC) curve method.

Methods

From January 2016 to January 2017, 81 patients with CVD were examined by emergency echocardiography in the Second Hospital of Shaoxing. The results of coronary angiography, intervention or surgical operation were regarded as gold standard, and those diagnostic efficiency indexes of emergency echocardiography for CVD were analyzed by ROC curve. The consistency between echocardiographic diagnostic results and gold standard results was analyzed. The difference between routine electrocardiogram examination and echocardiography diagnosis results were compared.

Results

Of the 81 cases, 67 were diagnosed positive by emergency echocardiography and 14 were negative. The diagnostic results of emergency echocardiography had moderate consistency with the gold standard (Kappa=0.764, P<0.01). There was no significant difference in the accuracy rate of the specific disease types (P>0.05). Sixty-one cases were positive by routine electrocardiogram, 20 were negative, and the diagnostic results were not consistent with the gold standard (Kappa=0.179, P>0.05). The ROC analysis showed that the diagnostic accuracy rate of echocardiography and routine electrocardiogram was 93.83% and 74.07%; the sensitivity was 94.29% and 78.57; the specificity was 90.91% and 45.45%; the misdiagnosis rate was 9.09% and 54.55%; the missed diagnosis rate was 5.71% and 21.43%; and the diagnostic area under curve (AUC) was 0.926 and 0.620, respectively. The diagnostic accuracy of emergency echocardiography for CVD was higher than that of routine electrocardiogram. There were no significant differences between echocardiography and routine electrocardiogram in test reaction time, examination time or diagnosis time (P>0.05).

Conclusions

Emergency echocardiography is of high diagnostic value for CVD, and has the obvious advantages of easy operation, quick diagnosis and high accuracy. It has great significance in the clinical diagnosis for CVD.
