目的

探究子宫动脉(UtA)血流动力学参数是否受多普勒超声成像位置差异影响。

方法

应用多普勒超声对孕鼠UtA 不同节段(UtA-1、UtA-2、UtA-3 和UtA-4:自宫颈向上的第一、第二、第三和第四妊娠囊旁)及其分支(UtB)、卵巢动脉(OA)进行观察成像,取得其血流动力学参数[收缩期峰值流速(PSV)、收缩期峰值流速与舒张末期流速比值(S/D)、搏动指数(PI)、阻力指数(RI)]数值,使用配对t 检验比较同一节段左、右侧血流动力学参数的差异,使用单因素方差分析比较不同节段血流动力学参数的差异,并采用LSD-t 检验进行两两比较。

结果

左、右侧子宫动脉不同节段、UtB、OA 测值比较,差异均无统计学意义(P 均>0.05)。UtA 不同节段各血流动力学参数差异有统计学意义(P 均<0.05),均呈逐渐下降趋势。当间隔至少一个节段时,UtA 各血流动力学参数显著下降(如PSV:UtA-2 段(32.94±11.24)cm/s vs UtA-4 段(25.93±13.10)cm/s,t=2.004,P=0.047;S/D:UtA-1 段(2.73±0.60) vs UtA-3 段(2.23±0.51),t=3.124,P=0.002;PI:UtA-1 段(1.06±0.21)vs UtA-3 段(0.84±0.26),t=2.819,P=0.006;RI:UtA-1 段(0.61±0.11) vs UtA-3 段(0.53±0.12),t=2.638,P=0.010)。UtB 血流动力学参数均明显低于UtA-1 段、UtA-2 段、UtA-3 段,OA 的血流动力学参数均高于UtB 段,差异均具有统计学意义(P 均<0.05)。

结论

多普勒超声成像在孕鼠UtA 不同节段所获得的血流动力学参数存在差异。基于孕鼠模型的子宫胎盘循环血流动力学参数研究应建立规范化扫查方法,以提高测量的一致性和准确性。

Objective

To investigate the influence of imaging location on uterine artery (UtA)hemodynamic parameters measured by Doppler ultrasound.

Methods

Doppler ultrasonography was used to assess the UtA at different segments (near the first [UtA-1], second [UtA-2], third [UtA-3], and fourth gestational sacs [UtA-4] ascending from the cervix), uterine artery branches (UtB), and ovarian artery (OA) in pregnant rats.Hemodynamic parameters such as peak systolic velocity (PSV), ratio of PSV to end-diastolic velocity (S/D),pulsation index (PI), and resistance index (RI) were obtained, and the difference in hemodynamic parameters between the left and right sides of the same segment was compared using the paired t-test. One-way ANOVA was used to compare the hemodynamic parameters among different segments of the artery, and the LSD-t test was used for pairwise comparisons.

Results

There was no significant difference in the measurements of different segments,branches, and OA of the bilateral uterine arteries (P>0.05). Uterine artery parameters differed significantly across segments (P<0.05), decreasing gradually. When measured >1 site apart, parameters showed significant reductions [eg, PSV: UtA-2 vs UtA-4 (32.94±11.24) cm/s vs (25.93±13.10) cm/s, t=2.004, P=0.047; S/D: UtA-1 vs UtA-3 (2.73±0.60) vs (2.23±0.51), t=3.124, P=0.002; PI: UtA-1 vs UtA-3 (1.06±0.21) vs (0.84±0.26),t=2.819, P=0.006; RI: UtA-1 vs UtA-3 (0.61±0.11) vs (0.53±0.12), t=2.638, P=0.010]. UtB parameters were lower than UtA-1/2/3 parameters (P<0.05). OA parameters were significantly higher than those of the UtB(P<0.05).

Conclusion

Doppler ultrasound yields varying hemodynamic parameters at different uterine artery segments in pregnant rats. A standardized scanning protocol is needed to improve consistency in uteroplacental circulation studies using pregnant rat models.
