目的

应用心脏做功指数(MPI)自动测量技术研究高危妊娠胎儿心脏功能的变化情况,探寻胎儿心功能简便易行的检测方法。

方法

选择孕龄在20~40周,单胎胎儿311例,用脉冲多普勒同时获得左心室流入道与流出道频谱信息,分别采用将测量线放在主动脉频谱峰值处一键获得左心室MPI的自动测量技术及分步测量等容收缩时间(ICT)、等容舒张时间(IRT)及射血时间ET后获得左心室MPI的传统测量法,其中异常胎儿(高危妊娠组)152例及正常胎儿(对照组)159例,比较不同测量方法获得的左心室MPI的差异性及两组间胎儿左心室MPI的变化程度。

结果

采用MPI自动测量技术获得正常对照组胎儿左心室MPI:0.510±0.040,高危妊娠组异常胎儿左心室MPI:0.542±0.124;结果显示,采用传统法测量获得正常对照组胎儿左心室MPI:0.504±0.055,高危妊娠组异常胎儿左心室MPI:0.538±0.123。运用MPI自动测量技术获得的高危妊娠组异常胎儿左心室MPI高于对照组正常胎儿,两组胎儿左心室MPI测值比较(t=-2.941,P<0.05),差异具有统计学意义,而两种测量方法获得的高危妊娠组异常胎儿左心室MPI测量结果基本相同。

结论

MPI自动法测量技术较传统方法更为简单、快捷,并具有良好的可靠性和重复性,易于在实际工作中推广应用,利于对胎儿心功能指标的探索。

Objective

To investigate the change of abnormal fetal cardiac function in high risk pregnancy by automatic measurement method of myocardial performance index (MPI), and explore the simple and easy method to assess fetal heart function.

Methods

Three hundred and eleven singleton with 20-40 gestational weeks were involved, Using pulse Doppler to obtain the spectral information of the left ventricular inflow tract and outflow tract, Automatic measurement of left ventricular MPI by using a key measurement line placed in the aortic spectrum peak and Measurement of isovolumic contraction time (ICT), isovolumic relaxation time (IRT) and ejection time (ET) by step by step to obtain the traditional measurement method of left ventricular MPI. Left ventricular MPI were measured in 152 abnormal fetus (high risk pregnancy group) and 159 normal fetus (control group). The differences of left ventricular MPI measured by the two methods and changes value of left ventricular MPI between two groups were compared.

Results

With MPI automatic measurement technique, the fetal left ventricular MPI were 0.510±0.040 in control group, and 0.542±0.124 in high risk pregnancy group, respectively; using the traditional method, the fetal left ventricular MPI were 0.504±0.055 in control group, and 0.538±0.123 in high risk pregnancy group, respectively. The fetal left ventricular MPI in high risk pregnancy group was higher than that in control group by using MPI automatic measurement technique (t=2.941, P<0.05).

Conclusion

MPI automatic measurement method may be more simple and accurate than the traditional method, which could have a better reliability and repeatability and be easy to apply in clinical practice.
