目的

分析儿童及青少年甲状腺癌超声声像图表现。

方法

选取2012年1月至2017年5月在江苏省中西医结合医院超声检查发现并经手术病理检查证实为甲状腺癌的儿童及青少年患者66例,分析儿童及青少年甲状腺癌超声声像图特征。

结果

术前超声显示:66例儿童及青少年患者中检出结节型甲状腺病灶56例(84.8%,56/66),伴微小钙化47例(83.9%,47/56),"毛刺状"改变44例(78.6%,44/56),边界不清晰37例(66.1%,37/56),颈部异常淋巴结14例(25.0%,14/56);另检出弥漫型甲状腺病灶10例(15.2%,10/66),伴微小钙化10例(100%,10/10);颈部淋巴结呈低回声,提示淋巴结转移7例(70.0%,7/10)。手术病理诊断:甲状腺乳头状癌64例(97.0%,64/66,包括10例弥漫硬化型乳头状癌),滤泡癌2例。

结论

超声早期诊断可对儿童及青少年甲状腺癌患者外科手术方式的选择及临床治疗提供有价值的诊断信息。

Objective

To analyze ultrasonographic features of thyroid carcinoma in childhood and adolescents.

Methods

In this study, the ultrasonographic features of 66 cases of children and adolescents who were diagnosed as thyroid carcinoma in the Department of Ultrasound of Jiang Su Province Hospital of Integrated Traditional Chinese and Western Medicine from January 2012 to May 2017 were retrospectively analyzed. All the thyroid nodules were confirmed by pathology.

Results

The results of preoperative ultrasound showed that in 66 cases of pediatric thyroid cancer, there were 56 cases of nodular type (55/66, 84.8%), 47 cases of microcalcification (47/56, 83.9%), 44 cases of irregular contour (44/56, 78.6%), 37 cases of unclear boundary (37/56, 66.1%), 14 cases of cervical lymph node metastasis (14/56, 25%). In 10 cases of diffuse type (15.2%, 10/66), there were 10 cases of microcalcification (10/10, 100%), 7 cases of hypoechoic cervical lymph node metastasis (7/10, 70%). Pathology confirmed 64 cases of papillary thyroid carcinoma (including 10 cases of diffuse sclerosing variant papillary carcinoma of thyroid) and 2 cases of follicular carcinoma.

Conclusion

Early diagnosis of ultrasound can provide valuable diagnostic information for the choice of surgical procedures and treatment for the patients.
