目的

应用超声心动图左心室压力-应变环(PSL)评价正常高值血压及高血压患者心肌做功。

方法

选取2019年8月至2020年3月于首都医科大学附属北京朝阳医院行超声心动图检查的高血压患者45例(高血压组),纳入同期正常高值血压患者40例(正常高值血压组)和正常血压受试者30例(正常血压组)。所有纳入对象均进行超声心动图检查。测量常规超声心动图参数,应用二维斑点追踪超声心动图测量受试者左心室整体长轴应变(GLS)、机械离散度(MD);通过左心室PSL评价心肌做功参数,包括心肌整体做功指数(GWI)、整体有效做功(GCW)、整体无效做功(GWW)、整体做功效率(GWE)。比较3组间超声心动图参数以及心肌应变和心肌做功参数,分析心肌做功参数与常规超声心动图参数和心肌应变参数的相关性。

结果

正常血压组、高值血压组、高血压组GLS呈减低趋势,但3组间差异无统计学意义(P>0.05)。高血压组较正常血压组、高值血压组MD增大,差异均有统计学意义(P均<0.05)。高值血压组较正常血压组GWI、GCW增加(P均<0.05)。高血压组较正常血压组GWI、GCW、GWW增加,GWE减低,差异均有统计学意义(P均<0.05);较高值血压组GCW、GWW增加,GWE减低,差异均有统计学意义(P均<0.05)。高血压组中,GWI、GCW分别与GLS(r=0.695、0.691,P<0.001)、LVEF(r=0.409、0.406,P=0.001)呈正相关。GWW与GLS呈负相关(r=-0.306,P<0.05),与MD呈正相关(r=0.384,P=0.002)。GWE与GLS呈正相关(r=0.519,P<0.001),与MD呈负相关(r=-0.482,P<0.001)。

结论

正常高值血压患者心肌做功效率仍正常,而高血压患者心肌做功效率受损,能量损失增加。超声心动图左心室PSL定量评价心肌做功可为正常高值血压及高血压患者心脏功能评估提供更多有价值的信息。

Objective

To evaluate myocardial work by echocardiographic left ventricular pressure-strain loop (PSL) in patients with high normal blood pressure and hypertension.

Methods

Forty-five patients with hypertension (hypertension group) who underwent echocardiography at Beijing Chaoyang Hospital Affiliated to Capital Medical University from August 2019 to March 2020 were selected retrospectively. Forty patients with high normal blood pressure (high normal blood pressure group) and 30 subjects with normal blood pressure (normal blood pressure group) were included in the same period and received echocardiography. The parameters of conventional echocardiography were measured, and the left ventricular global longitudinal strain (GLS) and mechanical dispersion (MD) were assessed by speckle tracking echocardiography. The left ventricular PSL was used to evaluate myocardial work parameters, including global work index (GWI), global constructive work (GCW), global waste work (GWW), and global work efficiency (GWE). Echocardiographic parameters, myocardial strain parameters, and myocardial work parameters were compared among the three groups, and the correlation of myocardial work parameters with conventional echocardiographic parameters and myocardial strain parameters was analyzed.

Results

GLS decreased successively in the normal blood pressure group, high normal blood pressure group, and hypertension group, but there was no significant difference among the three groups (P>0.05). MD was significantly higher in the hypertension group than in the normal blood pressure group and high normal blood pressure group (P<0.05). Compared with normal blood pressure group, GWI and GCW were increased (P<0.05) in the high normal blood pressure group. GWI, GCW, and GWW were increased, while GWE was decreased in the hypertension group (P<0.05) compared with the normal blood pressure group. GCW and GWW were increased and GWE was decreased in the high normal blood pressure group (P<0.05) compared with the high normal blood pressure group. In the hypertension group, GWI and GCW were positively correlated with GLS (r=0.695 and r=0.691, P<0.001) and LVEF (r=0.409 and r=0.406, P=0.001). GWW was negatively correlated with GLS (r=-0.306, P<0.05), and positively correlated with MD (r=0.384, P=0.002). GWE was positively correlated with GLS (r=0.519, P<0.001), and negatively correlated with MD (r=-0.482, P<0.001).

Conclusion

The myocardial work efficiency of patients with high normal blood pressure is still normal, while that of patients with hypertension is damaged and the energy loss is increased. Quantitative evaluation of myocardial work by left ventricular PSL may provide more valuable information for the evaluation of cardiac function in patients with high normal blood pressure and hypertension.
